How To Fill Out a Fish Slip

Click here to see an example of a completed fish slip

(revised January 2011)

Catch information is needed by the Department to assess fish stocks and manage fisheries. The following information is required on each slip. The numbers refer to the sample completed slip on the back of this page:

Fish slips are no longer required for landings from license categories ZN (rockfish), ZX, ZY (special use herring) or H (roe herring).

1. WHO THE FISH WERE OFFLOADED, SOLD, OR DELIVERED TO
  • The name and address of the buying company (the first buyer of the fish) if the catch is not groundfish.
  • The name and address of the offloading/unloading company if the catch is groundfish.
  • If this information is not pre-printed on the slip, please ensure that it is written legibly on the slip.
  • If the fish was not sold to a commercial buyer write Public Sale or Personal Use or other e.g. frozen for later sale, exported to the United States, used for bait, dumped or contaminated.
  • Report commercial sales, personal use, sales to the public etc. on separate fish slips.
  • The name of the plant, packer or collector to which to catch was unloaded from the catching vessel
  • The CFV number of the packing vessel, if applicable.
  • Ensure that you have a sufficient supply of slips on hand and do not need to borrow another company's slips. This usually results in buyer information reporting errors.
2. WHO FISHED
  • The VRN (vessel registration number) number and the name of the catching vessel. (VRN is the new name for CFV commercial fishing vessel)
  • The name and address of the vessel operator or license holder.
  • If the catch results from an aboriginal fishery, use an Aboriginal Fishery slip. Report the band name and fisher designation card number instead of a CFV. The landing slip number(s) for the landing slip corresponding to this catch must also be recorded.
  • If the catch is clams, a separate fish slip is required for reporting each licensed clam harvester. The clam license number must be recorded.
  • A fish slip is required for fish seized by the crown.
3. WHERE IT WAS CAUGHT
  • The areas of catch and the number of days fished in each area (see example).
4. HOW IT WAS CAUGHT
  • The type of gear used. Check the appropriate box or write the gear used.
5. WHEN IT WAS LANDED
  • The date the fish were landed (year, month, day) even if the catch is not immediately sold e.g. frozen for later sale. Report each landing date on a separate slip.
6. WHAT WAS CAUGHT
  • List all species separately (e.g. Harlequin rockfish, brill sole, halibut, chinook)
  • If the catch is salmon, report the pieces (number of fish) and weight (lb). For other species, report weight (lb).
  • The form in which the catch was landed (e.g. round, dressed, head-on, head-off, Japanese cut, iced, frozen)
  • The price ($ per lb) and value of the catch for each species and grade. If the catch was not sold, leave the price and value blank.
  • There are separate types of fish slips for both the commercial and the Aboriginal sales fisheries.
  • The use of computer software to print slips on a laser printer is an alternative to using preprinted fish slips. DFO can provide you with a standard fish slip template.
  • This information is required under provincial regulation and DFO licence conditions and is to be submitted within seven (7) days of landing to:
Fisheries and Oceans Canada
