Statutory declaration instructions

If a harvest log in which harvest information has been recorded is lost or destroyed, and no complete records of that harvest information survive, the licence holder shall submit a statutory declaration that the harvest log was lost or destroyed to DFO’s Commercial Salmon Catch Monitoring Unit (CSCMU). The carbon copies of all pages with harvest information would represent a complete record of harvest information. So if the original pages were lost, the carbon copies may be submitted instead and thus eliminate the need for a statutory declaration.

To make a declaration, first complete the top of the Statutory Declaration form. Next, visit with a commissioner for Oaths, Notary Public or Justice of the Peace to witness your signature on the bottom of the form. In order to expedite the licence clearance process, you may fax or email a copy of the signed Statutory Declaration to the CSCMU. You must then mail the original form to the CSCMU at the address below.

Do solemnly declare that

Due to circumstances beyond my control, the commercial salmon logbook, number____________________, was lost and therefore cannot be returned to the service provider

and I make this solemn declaration conscientiously believing it to be true and knowing that it is of the same force and effect as if made under oath and by virtue of the Canada Evidence Act.
