Instructions for using this Excel template
- Go to "Step 1 - General Info" worksheet.
- Select Licence Holder Name from drop-down list
- Then select “Facility Reference No" from drop-down list
- Complete the remaining columns (if applicable)
- Go to "Step 2 - Report Details" worksheet.
- Enter data and complete the remaining columns (if applicable)
- Explain in comments when “Other” is chosen from the pick list
- Data can be copied and pasted into multiple subsequent rows. However, please note that data can not be copied to cells marked "do not enter". *
Colour meaning

Guidance for reporting of marine megafauna incidents
Incident date - means the date on which the marine megafauna incident was discovered.
Incident time - means the time at which the marine megafauna incident was discovered.
Occurrence categoy -
- New - 24 hour - For notification of new marine megafauna incidents. Reports must include as much information as possible and be submitted within 24 hours of discovery, as per Condition of Licence.
- 7 day Follow-up - A complete report must be submitted to the Department within 7 days of the Incident Date, as per Conditions of Licence.
Fish on site? (Y/N) - Identify if there were cultured fish on site at the time of the incident.
Average weight (grams) - Identify the size of the cultured fish on site, if applicable.
Megafauna species (common name) - The common name of the species of marine megafauna involved in the drowning, entrapment or entanblement. Select species using the drop-down list provided. If a species is not listed, choose "Other" and enter the Common Name in the "Comments" column.
Incident type - Choose from the drop-down list:
- Drowning - means that the marine megafauna drowned and was found deceased as a result of the incident.
- Entrapment - means that the marine megafauna became entrapped within the facility, either in the net pens or within the perimeter of the predator net, but is/was swimming freely at the time of discovery.
- Entanglement - means that the marine megafauna became entangled in component of the facility infrastructure, but was released without drowning.
- Other - explain - a situation that does not fit the categories of drowning, entrapment or entanglement. Use the "Comments" column for further explanation.
Animal condition - Choose from the drop-down list:
- Fresh - Animal was recently deceased with little to no decomposition.
- Moderate-Advanced decomposition - Animal was deceased for an extended period of time with some level of decomposition.
- Live - Animal was found live and either released or attempted to be released from the containment array.
System component - Choose from the drop-down list:
- Containment net - animal was found within or entangled in a containment pen.
- Predator Net - animal was found between the predator net and the containment net, or entangled in the predator net.
- Other - animal was found entrapped or entangled in another system component. Use the "Comments" column for further information.
Photos taken? (Y/N) - Identify if photos were taken of the drowned, entangled or entrapped marine megafauna.
Carcass stored pending DFO advice (Y/N) - Identify if the carcass has been stored for DFO.
Animal released (Y/N) - Identify if the animal was released live.
Carcass discarded (Y/N) - Identify if the carcass was discarded.
Other action taken (Y/N) - Identify if any other actions were taken. Use the "Comments" section to provide more information.
Prior mitigation measures - Describe any mitigation measures that were in place prior to the incident that were meant to prevent entrapment or entanglement of marine megafauna.
Incident cause - Describe, with as much detail as possible, the cause of the incident and the events leading up to the incident.
Corrective measures taken - Describe any future mitigation measures that have been or will be taken to prevent further incidents from occuring.
Comments - Any additional comments pertaining to the marine megafauna incident, or describing any "other" categories listed above.
